Sometimes a the lab and MRO need additional information to determine whether a drug test is positive or negative. And, of course, there are extra charges involved.

Service Description
D&L Isomer Testing The D&L test is an additional lab test to determine the specific positive methamphetamine metabolite(s). This test is required for completion of the medical review process and will be ordered without client authorization when it is not included by default.
6-MAM Testing The 6-MAM test is a test for the presence of heroin. This test is required for completion of the medical review (MRO) process and will be ordered without client authorization in the case of a morphine positive when it is not included in the drug test panel.
THC-V Testing This is an additional test to determine if a marijuana positive is due to a legitimate prescription source such as Marinol, Dronabinal or Sativax. For NONDOT cases, the client will be given the option to order this test.
Anabasine Testing This is an additional test that can be run on specimens that are tested for Cotinine to confirm current tobacco use. This test is done by client request.
Interpreter Fee When the donor cannot adequately speak or understand English, an interpreter is used to help conduct the donor interview or to assist a donor in the drug testing process.
Split Testing, DOT Split testing is offered during the medical review process. The donor has the option to have the sample sent to another SAMHSA- certified laboratory for reanalysis of the non-negative result. The charge for split testing will appear on the employer’s invoice. The employer does have the right to seek reimbursement from the donor for this test as outlined in part 40 of the DOT regulations: https://www.transportation.gov/odapc/part40/40-173
Split or Aliquot Testing, NONDOT Split testing is offered on all applicable tests during the medical review process. For NONDOT cases in which a split collection was not completed, the donor has the option to have an aliquot (portion) of the original sample sent to another laboratory for reanalysis of the non-negative result. For NONDOT tests, unless payment is authorized by the client, the donor will need to provide payment prior to the split specimen processing.
Split/Aliquot Testing When a split or aliquot specimen is ordered, there will be an administrative fee added to each test for processing and review.
Specimen Upgrade or Downgrade Admin Fee The administrative fee charged when the client requests that a NONDOT test be upgraded to a DOT test, or a DOT test be downgraded to a NONDOT test.
FMCSA Clearinghouse Admin Fee The administrative fee charged when the MRO is required to remove a report from the FMCSA Clearinghouse due to donor or client delays in communication.
MRO Litigation Package, NONDOT The MRO Litigation Package includes the MRO result, the laboratory report, and a copy of the chain of custody form. The MRO notes are not included unless the client confirms that the case has gone to litigation, or we are subpoenaed for that information. Charged upon client request.
Lab Litigation Package The Lab Litigation Package includes a full breakdown of the specimen and QA testing processes completed while the specimen was at the lab. Charged upon client request.
Lab Reject This fee is charged to cover the lab accessioning fee and the MRO processing time when a specimen is rejected for testing by the laboratory due to an error or missing information. Collection fees will be reviewed on a case-by-case basis. If the specimen was rejected due to a collection site error, i3screen will not charge for the collection fee or pay the collection site for the service.
Shy Bladder Review This is the MRO review of the donor’s inability to provide a specimen at the time of collection. This process can take an extended period of time to complete, and the MRO may need to order additional services or testing. This is a DOT process and is not standard for NONDOT testing but is available upon request.
MRO Consultation, Hearing Prep, Expert Testimony This hourly charge is applicable if one of our MROs is needed for litigation.
MRO Travel Fee(s) These charges will apply if our MRO is required to travel to be onsite for any part of the litigation process. Telephonic or video testimony is always requested and preferred in lieu of travel.
