A Custody and Control Form, often called a CCF, documents the chain of custody for a specimen. This form exists in both paper and electronic formats (eCCF).

The paper version consists of five parts form, each serving a specific purpose.

  • The first page is the Lab Copy, which goes to the laboratory for testing.
  • The second page is designated for the Medical Review Officer (MRO) to review the results.
  • Employers receive the third page for their records.
  • The fourth page is used by the collector to document the collection process.
  • Finally, the last page is the Donor Copy, which the individual being tested keeps for reference.
