If an employee or candidate does not bring an acceptable photo ID, the employer’s Designated Employer Representative (DER) can verify their identity over the phone.
The donor must also have a test authorization form. This may include a Custody and Control Form (CCF), an electronic test authorization form, or an Authorization Form from InOut Labs.
