Sometimes, the lab and Medical Review Officer (MRO) need extra information to determine if a drug test is positive or negative. These additional drug testing services involve additional charges.

Additional Drug Testing Services and Charges

D&L Isomer Testing

The D&L test identifies the specific positive methamphetamine metabolite(s). This test is necessary for the medical review process. If the test is not included by default, the MRO will order it without client authorization.

6-MAM Testing

The 6-MAM test detects the presence of heroin in the sample. This test is essential for the MRO process and will be ordered automatically if the morphine test shows a positive result and it is not part of the drug test panel.

THC-V Testing

THC-V testing checks whether a marijuana positive result is due to a legitimate prescription, such as Marinol, Dronabinol, or Sativex. For non-DOT cases, the client has the option to request this test.

Anabasine Testing

Anabasine testing can confirm current tobacco use by detecting its presence in specimens tested for Cotinine. This test is performed at the client’s request.

Interpreter Fee

If the donor cannot understand or speak English, an interpreter will be needed. The interpreter helps conduct the donor interview and assists during the drug testing process.

Split Testing, DOT

Split testing is available during the medical review process. The donor can choose to send the sample to another SAMHSA-certified laboratory for reanalysis of a non-negative result. The employer will see the charge for split testing on their invoice. Furthermore, the employer can seek reimbursement from the donor for this test as outlined in part 40 of the DOT regulations: Part 40 Regulations

Split or Aliquot Testing, NONDOT

For non-DOT cases, split testing is available if the initial collection did not include a split. In this case, the donor may send a portion of the sample to another lab for reanalysis of a non-negative result. Unless the client authorizes payment, the donor must pay for the split specimen before processing.

Split/Aliquot Testing

When a split or aliquot specimen is ordered, an administrative fee will be added to cover processing and review of the test.

Specimen Upgrade or Downgrade Admin Fee

The administrative fee charged when the client requests that a NONDOT test be upgraded to a DOT test, or a DOT test be downgraded to a NONDOT test.

FMCSA Clearinghouse Admin Fee

The administrative fee charged when the MRO is required to remove a report from the FMCSA Clearinghouse due to donor or client delays in communication.

MRO Litigation Package, NONDOT

The MRO Litigation Package includes the MRO result, the laboratory report, and a copy of the chain of custody form. The MRO notes are not included unless the client confirms that the case has gone to litigation, or we are subpoenaed for that information. Charged upon client request.

Lab Litigation Package

The Lab Litigation Package provides a detailed breakdown of the specimen and QA testing processes completed while the specimen was in the lab. This breakdown is available upon client request and is charged accordingly.

Lab Reject

The lab charges a fee to cover the accessioning fee and MRO processing time when a specimen is rejected for testing due to errors or missing information. Collection fees will be reviewed on a case-by-case basis. If the lab rejects the specimen due to an error at the collection site, i3screen will not charge the collection fee or pay the collection site for the service.

Shy Bladder Review

The MRO will review the donor’s inability to provide a specimen during collection. This process may take an extended amount of time to complete, and the MRO may need to order additional services or testing. While this review is part of the DOT process, it is not standard for NONDOT testing. However, the review is available upon request.

MRO Consultation, Hearing Prep, Expert Testimony

An hourly charge applies if the MRO is required for consultation, hearing preparation, or expert testimony during litigation. The charge covers the MRO’s time and expertise.

MRO Travel Fee(s)

MRO travel fees apply when the MRO needs to travel onsite for any part of the litigation process. Telephonic or video testimony is always preferred over travel and will generally be requested in place of in-person attendance.
