Federal and Non-Federal CCFs look similar at first glance. A Federal Form, used for all DOT testing, displays “Federal Drug Testing Custody and Control Form” at the top. A Non-Federal Form shows “Forensic Custody and Control Form.” The laboratory’s name (Quest Diagnostics, LabCorp, Alere, CRL, MedTox, etc.) appears in the upper right corner.
