FMCSA Clearinghouse Query Request – Spreadsheet Upload
To InOut Labs: Please consider this authorization to conduct a Pre-Employment Full Query for the following job candidate(s). I understand that employees may NOT begin safety sensitive work until we have received a “driver not prohibited” query AND a negative drug screen for pre-employment. I also understand that if the driver is not registered with the FMCSA Clearinghouse that the results of the query may be delayed.
I understand that each query will deduct $1.25 from Employer’s Clearinghouse account. In the event the funds are not available then the query will be suspended until funds are available in Employer’s Clearinghouse account. I also understand that the 3-year history check remains a requirement until 1/6/2023.
I further understand that InOut Labs charges a fee of $10 for each query submitted on behalf of employer.
